Requirements: 

• Earn a UK cumulative GPA of 3.0 or higher 
• Earn a DHN Major GPA of 3.0 or higher 
• Speak with your academic advisor to determine if you are eligible to begin Supervised Practice Program in January 
• Complete your ACP Application form 
• Secure two recommendations from the following: 
  • Faculty member or academic advisor 
  • Work or volunteer supervisor 
• Write a two-page personal statement See application form for details 
• Provide at least 3 possible blocks of time for interviews 
• Submit application packet at one time electronically or as a hard copy to elmars2@uky.edu by October 1 at 11:59 pm.
